You should summarise your key findings in both written and graphical formats before producing an evaluation ‘pack’.
Share your findings with the relevant audience and stakeholders

Your report is now ready to be shared with stakeholders. You should also consider sharing results with patients if they were part of the evaluation process.
Use your findings to make decisions and sustain your initiative

Your report has been submitted but your work doesn’t necessarily end there. If your initiative has shown positive impact and if it is continuing after your evaluation has been completed, you should still consider how to monitor it in the longer term.
Follow up on your findings and monitor your initiative’s ongoing performance

If your initiative has shown positive impact and you want to scale it and/or transfer it to business as usual it is important that you continue to observe and monitor performance.
